Design Thinking

Design thinking is a set of methods and tools that combines empathy, creativity and rational analysis to meet users' needs. Organizations can use it both to improve internal processes and to solve customer pain points in new ways.

Lean Manufacturing & Safety

Brings the Lean principles of eliminating waste and continuous improvement into safety management. Standardized work, visual management and structured problem solving also become tools for identifying and controlling risks in day-to-day operations.

National Safety Council

A U.S. nonprofit organization founded in 1913 and dedicated to preventing avoidable injuries and deaths at work, on the road and in communities. A reference for safety research, data and good practices adopted by companies around the world.

HAZOP

Hazard and Operability Study. A structured process hazard analysis technique that examines, node by node, deviations from design conditions (more, less, none, reverse) to identify causes, consequences and safeguards. Widely used in the chemical, petrochemical and continuous process industries.

FMEA

Failure Mode and Effects Analysis. A systematic method for identifying failure modes in equipment, processes or systems, assessing their effects and prioritizing actions based on severity, occurrence and detection.

HECA

Hazardous Energy Control Assessment. An assessment of exposure to hazardous energy and of whether direct controls are in place to prevent serious injuries and fatalities, based on the energy-based safety approach.

OSHA

Occupational Safety and Health Administration. The U.S. Department of Labor agency responsible for setting and enforcing occupational safety and health standards. Its references, such as the Process Safety Management program, guide practices in many countries.

CSRA

Critical Safety Risk Assessment. An assessment of an operation's critical risks, those with the potential to cause serious injuries and fatalities, that verifies how effective the critical controls are and who is responsible for maintaining them.

Bowtie

A visual representation of a risk scenario built around a central event. On one side it connects threats and preventive barriers; on the other, consequences and mitigation barriers. It makes risks easier to communicate and barriers easier to track.

Causal Factor Analysis

Investigation methods that look beyond the immediate cause to identify the organizational, human and technical factors that contributed to the event. They lead to corrective actions that address the system, not just individual behavior.

Human Factors

The study of how people, tasks, equipment and work organization interact. It recognizes human error as a consequence of system conditions, not their cause, and guides the design of processes, interfaces and controls that keep work safe even when people make mistakes.

ISO 45001

ISO 45001 is an international standard for Occupational Health and Safety Management Systems (OHSMS), focused on improving an organization's performance in preventing work-related injuries and illnesses.

What If?

“What If” is a general, qualitative analysis technique used for a first pass at identifying risks. It can be applied in different areas of a company and is commonly used by planning teams to anticipate scenarios and alternatives for each of them.

Hearts and Minds

The Hearts and Minds toolkit was developed by Shell E&P, based on 20 years of university research, and is being applied successfully in Shell and non-Shell companies around the world. Hearts and Minds uses a series of tools and techniques to help an organization involve all employees in HSE management as an integral part of its business. Together, these tools and techniques are known as the Hearts and Minds Toolkit.
